• This application form only applicable to the guest(s) who
  had bought our travel related products (such as air ticket,
AIG Journey Safe Application Form
   packages, hotel….etc).
   
• After you have completed this application form, please
    email to enquiry@pattersontravel.com.hk
    or fax to 2868 4213, we will handle your application on our
    next working day, the policy number with policy wordings
    will be email or fax to you afterwards.
     
  • The prices listed on the application form are for quotation
    reference only. For confirmation, please refer to our
    company and American  International Underwriters, Ltd.
